Step 2: Obtain an affidavit from the local notary and contact the officer regarding the change of name in the birth certificate. However, if you want to change your parents' name or date of birth, supporting documents will vary. For example, in Bihar you would need a surname changing deed. In some cases, you may be required to provide a discharge certificate from the hospital, often asking for a change in the date of birth. Make an affidavit on minimum non-judicial stamp paper and mention the reason for change of name.

Step 3: Fill the Correction Form (Birth Certificate Correction) and submit it to the Municipal Corporation/Gram Panchayat along with the affidavit or attested copy with the original signature of the Judge.

Step 4: After your application is approved by the issuing authority, you will have to publish a short advertisement in the local newspaper as a public announcement. The format of the advertisement is given below:


I, XYZ have changed my name (Birth Certificate Correction) to ABC and will be known as ABC from here on out. I have submitted an affidavit to this effect, which has been signed.

Step 5: Publication in Official Gazette: This is the last step of the name-change (birth certificate correction) process. For your name to be legally recognized, you must publish it in the State Gazette (for example 'Delhi Gazette'). is.
